Good discharge management guarantees seamless follow-up care for patients. We focus on individual patient care requirements, and in the process also fulfill all the legal stipulations set out in the basic agreement for discharge management.
On your admission to hospital, we will consult with you about the amount of post-hospital care you are expected to require. Once we have this information, we will ask for your written consent so that we can manage your discharge from our hospital and transfer all the necessary information to the institutions providing further care (family physician, transferring specialist, rehabilitation clinic...). Your ward physician will inform you of the exact date of discharge, depending on your therapeutic progress.
Your expected requirement for follow-up care will be ascertained interdisciplinarily by physicians, nurses and our welfare service. The necessary follow-up measures need to be organized early enough, and the physicians or institutions providing follow-up treatment informed in good time. Our welfare service can help you, for example, to organize nursing care at home, rehabilitation, and the provision of medication and/or medical aids.
We will give you detailed information and advice about the discharge procedure, as well as your further treatment following discharge from our hospital. At the end of your stay, you will be given your discharge report and further documentation as appropriate.
